You don’t have to use up all your energy to write articles and meet deadlines. This makes you less tired and lessens the chance of you losing interest in everything else. It also helps exercise your mind and improves your line of thinking in the long run. 3) More fun than notWhile most people don’t find their line of work fun, the same cannot be said for freelance writing. Being a freelance writer helps you survive a recession in a fun way. Freelance writing jobs can be an exciting educational experience. You’ll learn a lot about the topic you’re writing about - be it automobiles, fashion, travel or something new. Aside from being given creative free will, you’re also able to attain more knowledge about a particular topic. Before you know it, you’re already an expert on the different kinds of car models and engines! There are many ways on how to survive a recession. However, not all of them can be as convenient - and as fun - as freelance writing. Even if you don’t think you can write well, I strongly suggest you try it out and take every opportunity that comes your way.

มะเร็งผิวหนังเป็นมะเร็งที่พบได้น้อย ประมาณร้อยละ 5 ของมะเร็งทั้งหมด มักพบในผู้ที่มีอายุมาก กว่า 40 ปี และพบในเพศชายมากกว่าเพศหญิง
ปัจจัยที่เป็นสาเหตุของมะเร็งผิวหนัง
1. แสงอัลตราไวโอเลต (UVA,UVB) พวกที่ต้องทำงานกลางแดด เล่นกีฬากลางแจ้ง ชอบอาบแดด จะมีความเสี่ยงสูงต่อการเกิดมะเร็งผิวหนัง
2. เชื้อชาติ คนผิวขาว ผมสีบลอนด์ ผิวไหม้แดดง่าย มีโอกาสเสี่ยงสูง เพราะมีเม็ดสีที่ผิวหนังน้อย ความสามารถในการป้องกันเซลล์ผิวหนังจากแสงอัลตราไวโอเลตจึงน้อยกว่าคนผิวคล้ำ คนที่เป็นโรคผิวหนัง Albinism ซึ่งมีความผิดปกติของการสร้างเม็ดสี จะพบมะเร็งผิวหนังได้บ่อย
3. การได้รับสารเคมีก่อมะเร็ง เช่น สารหนูที่ปนอยู่ในน้ำ ยาหม้อ ยาไทย ยาจีน ยาลูกกลอน
4. แผลเป็นจากไฟไหม้ น้ำร้อนลวก แผลจากผื่นผิวหนังบางโรค เช่น DLE
5. มีประวัติคนในครอบครัวเป็นมะเร็งผิวหนัง
6. เชื้อไวรัสบางชนิด เช่น HPV ที่ทำให้เกิดหูดที่อวัยวะเพศ
7. ภาวะภูมิคุ้มกันบกพร่อง เช่น ได้รับยากดภูมิคุ้มกันหลังการปลูกถ่ายไต
8. ผิวหนังในบริเวณที่เคยได้รังสีรักษา
9. คนที่สูบบุหรี่นานๆ จะเกิดมะเร็งในช่องปากได้

การป้องกันและรักษา
มะเร็งทุกชนิด ไม่ว่าจะเป็นที่อวัยวะใด ถ้าสามารถตรวจพบตั้งแต่แรก และกำจัดเซลล์ที่ผิดปกติออกได้หมด ก็สามารถหายขาดได้ มะเร็งผิวหนังมีข้อเด่นคือ ผิวหนังเป็นอวัยวะที่ห่อหุ้มอยู่ภายนอก เราสามารถมองเห็นได้ จึงทำให้สามารถตรวจพบความผิดปกติในระยะเริ่มแรกได้รวดเร็ว และยังติดตามการรักษาได้ง่าย
คนปกติทั่วไป จะมีเม็ดไฝ ขี้แมลงวัน เกิดขึ้นตามผิวหนังได้ทุกช่วงอายุ พออายุมากขึ้นก็มีพวกกระ หูด ติ่งเนื้อต่างๆ เราจะทราบได้อย่างไรว่า รอยโรคใดอาจเป็นมะเร็งผิวหนัง



อาการที่บอกว่าอาจเป็นมะเร็งผิวหนัง
1. ไฝที่เป็นอยู่เดิม มีรูปร่างเปลี่ยนไป อาจใช้หลักง่ายๆ คือ ABCD ดังนี้ A= ASYMMETRY ลักษณะของไฝทั้งสองข้างไม่เหมือนกันB=BORDER IRREGULARITY ขอบของไฝไม่เรียบC=COLOR สีของไฝไม่สม่ำเสมอD=DIAMETER ขนาดของไฝใหญ่กว่า 6 มม.
2. มีผื่นหรือก้อนที่เกิดขึ้นใหม่ และไม่หายใน4-6 สัปดาห์
3. ไฝหรือปานที่โตเร็ว และรูปร่างเปลี่ยนไปจากเดิม มีอาการคัน แตกเป็นแผล และมีเลือดออก
4. แผลเรื้อรังไม่หายใน 4 สัปดาห์

Breast Cancer Stages: Importance of Knowing Breast Cancer Stages

Determining the breast cancer stages is important since it enables the patient and doctor to identify the treatment necessary for one’s condition. Also, it is essential in assessing the risk of the given condition and what lifestyle changes the patient can do to improve their health.
Identifying A Breast Cancer's Stage
When talking about breast cancer stages, it is aimed at describing the extent of the cancer in the body. So, if you ask how a breast cancer is staged, doctors often start to classify whether it is invasive or non-invasive. Other factors considered are the tumor size, number of nymph modes involved, and what other parts of the body it has managed to affect.
Determining a cancer's stage is helpful during prognosis and deciding on a treatment option.
To determine the stage, a few standard procedures are done by the doctor on a patient. They undergo physical exam and biopsy to acquire the data needed by the doctor for the diagnosis.
If needed, further tests are also conducted such as imaging tests that include x-ray, bone scans, mammograms for the breasts, CT scans, positron emission tomography (PET), and magentic resonance imaging.
What Are the Breast Cancer Stages?
Now that the importance of determining the different breast cancer stages have been clarified, as well as the methods used to identify them, it is now time to move on to discussing each of the stages. Take note of the features and extent of the cancer in each of the stages:
Breast Cancer Stage 0
This stage renders the case of breast cancer to be non-invasive. At this point of the breast cancer, cancer or non-cancerous cells cannot be detected yet.
The abnormal cells are still at the stage wherein they try to spread out within the specific part of the breast where the cells are rooted. Also, they can try to expand on the neighboring tissues as the cancerous cells continue to grow.
Breast Cancer Stage I
Once the breast cancer enters this stage, it is now categorized as an invasive type of breast cancer. Meaning, the cancer cells have now worked their way towards the neighboring tissues. Stage I breast cancer also exhibit the following characteristics:
• The cancerous tumor has reached the size of 2 centimeters. • No lymph modes are affected.
Breast Cancer Stage II
For this particular stage of breast cancer, it is also known as an invasive type of cancer and is broken down into two more categories:
1) Stage IIA
Even in this particular stage, the conditions can be different:
• A tumor does not exist in the breast but cancerous cells are detected in the lymph nodes.• A tumor could exist but measures less than 2 centimeters;• The tumor has expanded beyond 2 centimeters but less than 5 centimeters without reaching the lymph nodes.
2) Stage IIB
This invasive level of the cancer are recognized as either one of the following:
• The tumor exceeds 2 centimeters in size but less than 5 centimeters, while also reaching the lymph nodes.• The tumor is more than 5 centimeters in size but has not yet reached the axillary lymph nodes.
Breast Cancer Stage III
1) Stage IIIA
In this stage, the tumor could either be detected or not. Aside from the axillary lymph nodes, cancer can also stick to other structures outside of the lymph nodes and become clumped together.
2) Stage IIIB
In this stage, the tumor can grow in size and affect other areas of the body outside of the actual breast, whether th chest wall or skin of the breast. This is the stage wherein inflammatory breast cancer takes place.
3) Stage IIIC
In some cases, sign of breast cancer might not be detectable yet. However, the tumor could already be spreading towards the breast skin, chest wall, and below your collarbone.
Breast Cancer Stage IV
In this level, the cancerous cells have managed to spread to various organs of the body. Therefore, the cancer is no longer restricted on the breast and lymph nodes, which signifies the initial diagnosis of breast cancer. The reason why diagnosis is done only during this stage is because cancerous cells were not detected while still within the breast.
Recognizing breast cancer stages does more than just identifying treatment options, but also enables doctors and patients to understand the developmental pattern of the disease.

ทั้งนี้ ในการทดลองที่โคเฮนร่วมกันทำกับทีมนักวิจัยของศูนย์โรคมะเร็ง เอ็ม.ดี.แอนเดอร์สัน แคนเซอร์ มหาวิทยาลัยเท็กซัส ในสหรัฐอเมริกา ได้ศึกษาในกลุ่มผู้ป่วยมะเร็งเต้านม 61 คนที่ผ่านการผ่าตัด และกำลังอยู่ระหว่างขั้นตอนรับการฉายรังสีเป็นเวลา 6 สัปดาห์ โดยแบ่งผู้ป่วยออกเป็น 2 กลุ่ม ให้กลุ่มผู้ป่วย 30 คนต้องเข้าคลาสเล่นโยคะสัปดาห์ละ 2 ครั้ง ส่วนกลุ่มที่เหลือไม่ต้องเล่นโยคะ
หลังจากนั้น พอครบ 6 สัปดาห์ ทีมงานวิจัยก็จะให้กลุ่มผู้ป่วยทั้งหมดตอบแบบสอบถาม ที่จะถามรายละเอียดต่างๆ อาทิ ความสามารถในการยกข้าวของ เครื่องใช้,ความสามารถในการเดินเป็นระยะทาง 1.6 กิโลเมตร รวมทั้งความสามารถในการทำกิจกรรม หรือสิ่งต่างๆ แล้วยังเรื่องความรู้สึกเหนื่อยล้า อ่อนเพลีย, อารมณ์ ความรู้สึก รวมทั้งเรื่องของคุณภาพชีวิตต่างๆ โดยตั้งคะแนนของแบบสอบถามมีตั้งแต่ 1-100 คะแนน
แล้วผลลัพธ์ที่ทีมวิจัยพบก็คือ กลุ่มผู้ป่วยที่เล่นโยคะจะได้ "คะแนนสูง" ในเกือบทุกเรื่องทุกหัวข้อ แทบจะสรุปได้ว่าประสิทธิภาพความสามารถทางร่างกายของผู้ป่วยได้คะแนนสูงถึง 82 คะแนน ขณะที่กลุ่มไม่เล่นโยคะได้คะแนนเพียง 69 คะแนน
ทั้งนี้ ผลการทดลองบอกว่า กลุ่มผู้ป่วยที่เล่นโยคะต่างบอกว่าพวกเธอรู้สึกเลยว่า สุขภาพโดยทั่วไปดีขึ้น รู้สึกอ่อนเพลียน้อยลง และหลับช่วงกลางวัน ไม่บ่อยนัก
แต่ในเรื่องของความรู้สึกซึมเศร้า หรือวิตกกังวล ทีมงานเล่าว่าไม่พบความแตกต่างใดๆ ระหว่างกลุ่มผู้ป่วยทั้งสอง

โคเฮนและทีมงาน ซึ่งได้นำเสนอผลงานวิจัยของพวกเขาในงานประชุมสัมมนาทางการแพทย์ จัดขึ้นเมื่อไม่นานที่เมืองแอตแลนต้า โดยสมาคม American Society of Clinical Ontology เล่าว่า เขายังได้เจาะเลือดและขอตัวอย่างน้ำลายของกลุ่มผู้ป่วยทั้งสองกลุ่มไปแล้ว เพื่อจะนำไปตรวจดู การทำงานของระบบภูมิคุ้มกันโรคภายในร่างกาย และวัดดูระดับความเครียด แต่ขณะนี้ผลการตรวจยังไม่เสร็จ ส่วนในอนาคต เขาก็เตรียมจะทำการทดลองขั้นต่อไป โดยจะเปรียบเทียบดูผลระหว่างการยืดเส้นกับโยคะ ว่าให้ผลลัพธ์ที่แตกต่างกันหรือไม่ อย่างไร โดยจะแบ่งกลุ่มคนไข้ให้อีกกลุ่มเล่นยืดตัว ยืดเส้น อีกกลุ่มก็เล่นโยคะไป ในช่วง 2-3 ปีที่ผ่านมา ได้มีการทดลองศึกษาอยู่หลายชิ้นที่พยามยามจะดูประโยชน์ของการเล่นโยคะที่มีต่อผู้ป่วยโรคมะเร็ง โดยเฉพาะการเล่นโยคะแบบทิเบตอย่างทีมของโคเฮน เมื่อเร็วๆ นี้ก็ได้เงินสนับสนุนก้อนใหญ่มีมูลค่าถึง 2.4 ล้านดอลลาร์จากสถาบันโรคมะเร็ง The National Cancer Institude เพื่อให้ศึกษาถึงผลลัพธ์ของการเล่นโยคะแบบทิเบตที่มีต่อหญิงผู้ป่วยมะเร็งเต้านมที่ผ่านการผ่าตัดและกำลังอยู่ระหว่างการรักษาด้วยการให้ยาหรือเคมีบำบัด มาถึงบรรทัดนี้ อาจมีหลายคนรู้สึกอยากฟังความรู้สึกของผู้ป่วยมะเร็งเต้านมที่ผ่านการเล่นโยคะดูบ้างว่าเธอรู้สึกอย่างไร เรื่องนี้ เทเรซิต้า ลาดริลโล วัย 52 หญิงชาวเมืองฮุสตันที่มาเข้าคลาสโยคะที่ศูนย์โรคมะเร็ง เอ็ม.ดี.แอนเดอร์สัน เล่าว่าการยืดเส้นยืดกล้ามเนื้อ ช่วยให้แขนขวาของเธอที่ยึดตึง เนื่องจากแผลที่เกิดจากการผ่าตัด สามารถยืดงอได้ดีขึ้น แล้วการฝึกควบคุมลมหายใจก็ช่วยให้เธอรู้สึกสงบ และนอนหลับได้ดี

เตือนภัย กินแล้วนอน ... โรคกรดไหลย้อน อาจถามหา

กรดไหลย้อน คือ ภาวะที่น้ำกรดในกระเพาะอาหารไหลย้อนขึ้นมาในหลอดอาหาร และในบางรายอาจไหลย้อนขึ้นมาถึงคอ และกล่องเสียงได้ปัจจุบันพบว่า ผู้ป่วยโรคนี้มีจำนวนเพิ่มมากขึ้นเรื่อยๆ ซึ่งอาจเป็นเพราะลักษณะรูปแบบการดำรงชีวิตของคนในสังคมเปลี่ยนไป เกิดความเครียด มีความเร่งรีบในการทำงานทำให้ผู้คนนิยมรับประทานอาหารจานด่วนที่อุดมไปด้วยไขมัน คาร์โบไฮเดรต และแคลอรี่สูง รวมทั้งการแพทย์ที่ก้าวหน้า และเครื่องมือที่ทันสมัยทำให้สามารถตรวจ และวินิจฉัยโรคนี้ได้มากยิ่งขึ้นด้วยภาวะนี้เกิดเนื่องจากพยาธิสภาพของกล้ามเนื้อหูรูดระหว่างหลอดอาหาร และกระเพาะอาหารทำงานไม่ปกติ อาการสามารถเกิดได้ทั้งกลางวันและกลางคืน ผู้ป่วยที่เป็นโรคนี้ในระยะเริ่มต้นอาจไม่รู้สึกว่ามีความผิดปกติหรือมีอาการแต่อย่างไร และผู้ป่วยบางรายอาจไม่เคยมีอาการของโรคกระเพาะหรือรักษาโรคกระเพาะมาก่อนเลยก็ได้น้ำกรดจะระคายเคืองหลอดอาหารทำให้เยื่อบุอาหารเกิดการอักเสบ ผู้ป่วยจะเจ็บในอก รู้สึกแสบร้อนในอกได้โดยเฉพาะเวลาเรอ นอกจากนั้นกรดยังสามารถระคายเคืองกล่องเสียงและคอหอยได้ด้วย ซึ่งอาการที่บริเวณคอหอยและกล่องเสียง คือ
1.เสียงแหบเป็นๆ หายๆ เรื้อรัง โดยเฉพาะเสียงแหบในเวลาเช้า
2.รู้สึกขมในปากและคอหลังจากตื่นนอนใหม่ๆ
3.คอและกล่องเสียงอักเสบบ่อยๆ รักษาหายได้ไม่นานก็กลับมาเป็นใหม่อีก
4.ระคายคอ และกระแอมบ่อยๆ รู้สึกว่าคอไม่โล่ง
5.ไอเรื้อรัง แต่พบว่าปอดปกติดี
6.กลืนอาหารลำบาก กลืนติดๆ กลืนไม่ลง กลืนแล้วเจ็บในคอ
7.ผู้ป่วยอาจรู้สึกเหมือนมีก้อนอะไรจุกๆ ในคอ ลมหายใจมีกลิ่น มีกลิ่นปาก
8.มีเสมหะในคอจำนวนมาก
9.รู้สึกว่าเหมือนมีเสมหะไหลลงคออยู่เรื่อยๆผู้ป่วยบางท่านอาจมีแค่อาการใดอาการหนึ่ง ในขณะที่บางท่านอาจมีหลายๆ อาการร่วมกันได้ อาการต่างๆ เหล่านี้อาจทำให้ผู้ป่วยเข้าใจผิดคิดว่ามีเนื้องอก หรือก้อนมะเร็งในคอ ทั้งนี้เมื่อทำการตรวจวินิจฉัยแล้วแพทย์ไม่พบก้อนเนื้อเหล่านั้นเลย กรณีนี้ทำให้ผู้ป่วยไม่สบายใจ วิตกกังวลและยิ่งเกิดความเครียดมากขึ้น


ปัจจัยเสี่ยงที่ทำให้เกิดภาวะกรดไหลย้อน คือ


โรคกระเพาะที่ไม่ได้รับการรักษาที่เหมาะสม และต่อเนื่อง
การสูบบุหรี่
การดื่มเครื่องดื่มที่มีแอลกอฮอล์เป็นประจำ
การดื่มเครื่องดื่มที่มีคาเฟอีน เช่น ชา กาแฟ ช็อคโกแลต


การรับประทานอาหารที่มีไขมันสูง รสจัด หรือมีส่วนประกอบของมะเขือเทศในปริมาณมาก
การเข้านอนหลังรับประทานอาหารเสร็จ 2 -3 ช.ม.
ภาวะโรคอ้วน มีน้ำหนักตัวเกินมาตรฐาน
การสวมเสื้อผ้าที่คับแน่น ก็มีผลทำให้เกิดโรคนี้ได้
การรับประทานยาบางชนิด เช่น ยานอนหลับ ยาคลายกล้ามเนื้อ ยาหวัดบางชนิด เป็นต้น


การตรวจวินิจฉัยภาวะกรดไหลย้อนนั้นแบ่งได้เป็น 2 อย่างด้วยกัน คือ การตรวจด้วยกระจกที่ใช้สำหรับตรวจกล่องเสียงและคอโดยเฉพาะ และการตรวจด้วยการส่องกล้อง ซึ่งการส่องกล้องนั้นแยกได้ออกเป็น 2 ส่วนด้วยกัน คือ


ส่วนที่ 1 คือ การส่องกล้องเพื่อตรวจดูตั้งแต่ลำคอจนถึงกล่องเสียง
ส่วนที่ 2 คือ การส่องกล้องเพื่อดูหลอดอาหารและกระเพาะอาหาร

การส่องกล้องดูลำคอและกล่องเสียงนั้น สามารถทำได้ที่ห้องตรวจผู้ป่วยนอก ใช้เวลาไม่นาน ไม่จำเป็นต้องใช้ยาสลบ และผู้ป่วยไม่จำเป็นต้องเตรียมตัวอะไรมา (ไม่ต้องงดน้ำและอาหาร)
ส่วนการส่องกล้องดูหลอดอาหารและกระเพาะอาหารนั้น ผู้ป่วยต้องเตรียมตัวโดยงดน้ำและอาหารมาก่อน เนื่องจากต้องมีการใช้ยาชา หรือวางยาสลบ และเพื่อที่จะได้ไม่มีเศษอาหารในกระเพาะอาหารมารบกวนขณะที่ทำการส่องกล้อง
ในผู้ป่วยบางรายพบว่ามีอาการในอกคล้ายกับผู้ป่วยโรคปอด โรคหัวใจ และมะเร็งบางชนิด จึงทำให้อาจต้องมีการตรวจพิเศษเพิ่มเติม เพื่อช่วยในการวินิจฉัยแยกโรค เช่น การกลืนแป้ง การเอ็กซ์เรย์ หรือเอ็กซ์เรย์ คอมพิวเตอร์ ซึ่งการตรวจเพิ่มเติมเหล่านี้ก็ไม่ก่อให้เกิดความเจ็บปวด หรือทรมานต่อผู้ป่วยแต่อย่างใด
สำหรับแนวทางในการรักษาภาวะกรดไหลย้อนนั้น แบ่งออกได้เป็น 3 แนวทาง

แนวทางที่ 1 : แพทย์จะให้คำปรึกษาและแนะนำในการเปลี่ยนแปลงพฤติกรรมบางอย่างที่มีผลต่อโรคนี้ เช่น ในผู้ป่วยที่สูบบุหรี่แพทย์จะแนะนำให้งดสูบ ในผู้ป่วยที่ชอบรับประทานอาหารปริมาณมากๆ แพทย์จะแนะนำให้รับประทานอาหารแค่พอดี ไม่ควรรับประทานอาหารจนอิ่มเกินไป ผู้ป่วยโรคอ้วน หรือมีน้ำหนักตัวมากแพทย์จะแนะนำให้งดอาหารรสจัดจำพวกเผ็ดและเปรี้ยว รวมทั้งอาหารที่มีไขมันสูงและเครื่องดื่มที่มีคาเฟอีน (ชา กาแฟ ช็อคโกแลต) และงดเครื่องดื่มที่มีแอลกอฮอล์ด้วย รวมทั้งลดอาหารที่มีส่วนประกอบของมิ้นท์ และมะเขือเทศจำนวนมาก

แนวทางที่ 2 : คือ แพทย์จะแนะนำให้ผู้ป่วยรับประทานยาเพื่อควบคุมการหลั่งของกรดในกระเพาะอาหาร ซึ่งในปัจจุบันยามีอยู่หลายกลุ่ม และกลไกการออกฤทธิ์แตกต่างกันไป ดังนั้น ผู้ป่วยจึงควรปรึกษาแพทย์ ก่อนใช้ยา และไม่ควรซื้อยาลดกรดรับประทานเอง เนื่องจากยานั้นๆ อาจไม่เหมาะสมกับสภาวะที่ผู้ป่วยกำลังเป็นอยู่

แนวทางที่ 3 : คือ ถ้าผู้ป่วยมีอาการมากขึ้นและไม่ตอบสนองต่อการรักษา 2 แนวทางแรก แพทย์จะแนะนำให้ทำการผ่าตัด ซึ่งเป็นการผ่าตัดให้กล้ามเนื้อหูรูดระหว่างหลอดอาหาร และกระเพาะอาหารนั้นกระชับขึ้น

ภาวะกรดไหลย้อนนั้น ควรได้รับการรักษาที่เหมาะสมและต่อเนื่อง การรับประทานยาเฉพาะเวลาที่มีอาการหรือเฉพาะตอนที่เป็นมากๆ มักไม่เพียงพอที่จะทำให้หายได้ และเมื่อรักษาจนอาการดีขึ้นแล้วก็ควรจะปรับเปลี่ยนพฤติกรรมต่างๆ ดังที่ได้กล่าวมาแล้วให้ต่อเนื่อง เพราะโรคนี้อาจย้อนกลับมาก่อความรำคาญให้ผู้ป่วยได้อีกเรื่อยๆ ถ้าผู้ป่วยยังมีปัจจัยเสี่ยงเหล่านั้นอยู่เมื่อใดที่ผู้ป่วยเป็นโรคนี้ ท่านไม่ควรนิ่งนอนใจหรือละเลยที่จะตรวจและรักษา หรือดูแลร่างกาย เนื่องจากพบว่าภาวะน้ำกรดไหลย้อนมีส่วนสัมพันธ์กับมะเร็งของกล่องเสียง และมะเร็งของหลอดอาหารด้วย โดยเฉพาะอย่างยิ่งในผู้ป่วยที่มีอายุมากกว่า 40 ปี และมีอาการของโรคนี้มานานเกิน 5 ปี
